fibroids?

I had a pelvic ultrasound today along with a follow up vaginal ultrasound to follow up on a 4 cm fibroid a while back. It is now 6 cm and the vaginal ultrasound was to see if another was behind this one. I have very heavy and painful periods and spot for a week before my period begins. I feel bloated, crampy and backache all of the time and have constipation and painful bowels. I fee that there is constant pressure on my bowel area. I have 3 children and am 40 so child bearing is done for me. After reading the options, I have no idea what to do. I just want to feel good again. I have no energy and just feel crappy all of the time.
